Thank you for submitting your insurance application. Once your application has been verified by PEF MBP for accuracy and eligibility, you will automatically be enrolled, and payroll deductions will commence within two payroll cycles.
PEF MBP staff may contact you by phone, email, or mail regarding your application. If there are errors or information is incomplete, you will receive instructions on how to correct your application. If you apply for coverage for which you are not eligible, you will receive an explanation of your ineligibility. Failure to respond to PEF MBP contact attempts, may impact our ability to process your application.
If you are within your first 240 days of employment in a PEF represented position, you are eligible to sign up for limited coverage amounts without completing any medical underwriting. If you are interested in higher amounts of coverage, please contact PEF MBP Insurance department to discuss your options.
If you have been employed in a PEF represented position for longer than 240 days, you must complete an Evidence of Insurability (EOI) form with Sun Life. The insurance company will approve or deny the application based on your EOI information. You may access the online EOI form via the button below. When completing your form, use:
